2 *Copy of PASSPORT (All Directors) Duly attested as "True Copy" by authorized signatory with Company Stamp.
3 *Bank Account Statement  for last 6 months Duly attested as "True Copy" by authorized signatory with Company Stamp.
4 *Audited Balance sheet & P & L statement for the last 2 years Duly attested as "True Copy" by authorized signatory with Company Stamp.
5 *Office Address Proof  Copy of your Landline telephone bill OR electricity bill duly attested as "True Copy" by authorized signatory with Company Stamp.
6 *Sales Tax registration OR Income Tax registration OR Municipal registration or Shop Establishment License Number OR Import Export Copy Duly attested as "True Copy"  by All Partners  with Company Stamp.
7 Your Business Card / Visiting Card  
8 *A Cancelled  Cheque For the bank account into which you will be receiving payments
9 *Signature and photograph verification letter on your  Company letterhead As per format
10 *Board Resolution on your  Company  Letterhead As per format
11 *A Letter of Authorization  on your company Letterhead As per format
12 * The Sub-Merchant Agreement (Please Note that: we accept only PDF FORMAT of the Agreement.) Franked for the value of Rs 300/- OR separately attach Non-Judicial stamp paper for the value of Rs 300/- with the following content written on it.

"This Non-Judicial Stamp Paper forms an integral part of the Avenues India Pvt Ltd. Internet Gateway Merchant Legal Agreement executed by ______________________ (Company name) on ____________(date)"
13 *Copy of Memorandum & Articles of Association, which include the Certificate of Incorporation
duly attested as” True Copy “ on 1st & last three pages  by  authorized signatory  with Company stamp.
14 *Latest list of all Directors with their Addresses If there are more/less directors other than mentioned in the MOA & AOA, then in that case, Form 32 duly attested as ” True Copy “by  authorized signatory  with Company stamp.
All documents marked with * is mandatory.

Note: a) The above mentioned documents are required to be signed by the signing authority who will be signing the Agreement, the signatory have to follow the PAN Card signature only on all documents.

b)Kindly ensure that the stamp paper is in your Company name, Sign and Stamp should be on the bottom right hand corner of each page, you should sign if you are entering into an agreement with us in your personal capacity and towards the end of the agreement in the column mentioned 'For and on behalf of the Sub Merchant' on 11th  page. Please do fill all the details in the Agreement. Kindly ensure that the relevant details in “clause g” for NEFT settlement and details in Annexure A are also filled out. (Please fill all the details manually)

Important: If you make any corrections on the agreement, you need to confirm the corrections with a signature.

c) Authorization letter, Board Resolution should be dated & the same should be less than 60days
